Embedding self management support through service redesign at Highgate Surgery

This case study briefly describes how the Highgate Surgery have adapted some of their ways of working – such as the diabetes template – to enable them to better support patients to self-manage.

Whilst the changes initially led to longer consultations over time not only have the length of consultations reduced back to the previous level, but it appears that patients are making fewer appointments annually. The initial changes have also led to further developments in ways of working within the practice.
