Goal setting is a key part of supporting people to self-manage. Developed as part of the Health Foundations Co-creating Health programme, this is a generic sheet which can be used by people with different long term conditions. It enables them to record a health-related goal they would like to set themselves and how they will achieve it on a week by week basis.
