This original paper discusses the importance of measuring harm in maternity services in order to fully understand the associated safety risks to both mother and baby.

It focuses on a data driven approach to safety using the example of the maternity safety thermometer. Importantly, it includes measures areas of harm which matter to patients which are often omitted from systematic measurement such as separation from baby and sense of psychological safety.
