This improvement report discusses work undertaken at Medway NHS Foundation Trust to reduce the incidence of pressure ulcers. The team involved designed a comprehensive database and investigated all acquired pressure ulcers through root cause analysis before creating an action plan.

The tissue viability nurses aligned this work to High Impact Actions (HIAs) and used innovative thinking to address and reduce the incidence of pressure ulcers.
