This poster is aimed at children and is designed to encourage and give them confidence to ask their health professional questions.

It sets out that the service and their health professional want to know what is important to them and for them to be involved in decisions about their care. It encourages patients to think about what they would like to ask in the consultation, and includes space for services to add an example question.
