The Compendium is aimed at commissioners as well as health and social care professionals to provide the evidence for improving care and outcomes for people with long term conditions.

It provides information and evidence to reinforce why a focus on LTCs should be a priority, including providing the latest statistical data on long term conditions. It sets out supporting and enabling people to self manage as one of five key outcomes for people with long term conditions.
