In healthcare, the word transition is used to describe the process of preparing, planning and moving from children’s to adult services.

To support young people through transition, the University of Southampton’s Childrens’ Hospital has developed the Ready Steady Go transition programme.

The resources include questionnaires and information packs for young people, families and healthcare professionals to help them assess and feel confident in their move to adult services.

It is supported by young adult cohorted clinics until age 25 in adult services.
