This case study briefly describes how the team in Calderdale and Huddersfield working on the Health Foundation’s Co-creating Health programme developed personalised postcards inviting people who had attended self-management training for patients to come to a re-union.
The Patient Skills Programme is designed to give people the knowledge, skills and confidence to self-manage. In Calderdale and Huddersfield the course is followed by re-union meetings of those who have attended.
To encourage people to attend and to support them to follow up the health goals they have set themselves, at the last session of the Patient Skills Programme participants write their goals on a postcard, which they then hand in to the tutor. When the team advertise the reunion, participants are sent their own postcard back two to three weeks before the meeting as reminder.
The postcards have been positively received by patients.
